Would this part work with a 2005 PT GT 5-speed Getrag transmission?

  No, the PT GT has a completely different transmission and it already has a hydraulic slave cylinder from the factory.

Will this work on an ACT 6 puck non organic?

  This will work on any regular Neon SRT-4 clutch that uses the fork and its also been tested with the Clutchmasters twin disc clutch. Its likely to work with other twin disc units like Spec, ACT Twin disc, etc.
